Health Assessment Form
Must be turned into the school nurse prior to try-outs. Physicals cannot expire during the season. Physicals are good for 13 months.

Medication Authorization Form
(for Inhalers, Epi Pens, etc.) Must be turned into the school nurse if there is medication that the student is required to take or carry.
